2008 Volunteer Form

We want to thank you for wanting to be part of our staff this year. We want this to be a win-win for both you and us. We are giving you a ticket to the event in exchange for your commitment to work a certain number of hours.

Please complete the information below and be sure to mark enough choices for us to schedule your hours. Some spots are limited and may or may not be available to work. Those registering early get priorioty over those registering closer to the event. We will try to accommodate your requests, but may not be able to.

We will be contacting you prior to preparing the schedule to verify you are still interested in working. You then will be sent a draft of the schedule for your review and approval around the first week of November.

We are offering 2 Volunteer options:

• $140 ticket value - 12 hours of work required (open seating/SRO and no Thur. Dance or workshops)
• $165 ticket value - 12 hours of work required, plus $50 (please contact the US Open home office 800-537-8937 to secure your reserved seat. This upgrade includes the Thursday night dance and workshops.)

We are scheduling everyone in four 3 hour shifts. (You may want to work back to back to work off hours faster)

Shift Hours and codes:

• M - Morning
• N - Noon
• A - Afternoon
• E - Evening
• L - Late
• V - Very Late
